CMS-1500 (version 08/05)
The National Uniform Claim Committee (NUCC) has released a revised CMS-1500 claim form that will accommodate NPI. The new form CMS-1500 (version 08/05) replaces CMS-1500 (version 12/90). During the dual-use period, CMS will accept either version of the form. Beginning July 1, 2007, CMS will accept only CMS-1500 (version 08/05).

UB-04
The National Uniform Billing committee (NUBC) has approved new form UB-04 to replace UB-92. The UB-04, also known as CMS-1450, will accommodate NPI. During the dual-use period, March 1, 2007 through May 22, 2007, CMS will accept both the UB-04 and the UB-92 . Beginning May 23, 2007, CMS will accept only the new UB-04 form.

ADA 2006
The American Dental Association (ADA) will release the ADA 2006 claim form to replace the ADA 2005. During the dual-use period, January 1, 2007 through May 22, 2007, either form will be accepted. Beginning May 23, 2007, only ADA 2006 will be allowed.
